First Visit Checklist

Upon arriving for your first visit please come prepared with the following items.

Patient Forms
  • Insurance information and card.
  • Photo ID.
  • All X-rays (film or disc) from other groups or hospitals.
  • A list of your current medications.
  • Information regarding previous treatments and medications.
  • Medical records from prior visits that you think may be helpful.
  • Download and print New Patient/History & Physical form.
